When a family asks AI for home care in Raleigh, it names national chains. Five of the top seven aren't local at all.

I ran the questions adult children actually ask through ChatGPT, Gemini and Perplexity in July 2026, across three senior-care markets. Everything below is what came back.

Your buyer is the daughter, not the resident.

The queries that surface your market aren't “near me” searches from a 78-year-old. They're research questions from a family member: how much does assisted living cost, home care vs assisted living, how to protect assets from a nursing home.

She is usually 50-something, often several states away, and frequently working inside a two-week window after a fall or a diagnosis. She has no prior knowledge of your market and no time to build any.

So she asks an assistant, and takes the three names it gives her. That is the entire shortlist.

The chains are winning the answer

Fifty-three agencies were named at least once. The two most-cited are national brands, and five of the top seven are franchises. The median agency appeared in 10% of answer slots — and 40 of the 53 were named exactly once.

Why this one matters most

A franchise does not have better care than you. It has a national content operation, consistent structured data across hundreds of locations, and directory listings the engines already trust.

None of that is clinical. All of it is fixable, and it is the reason a family three states away has never heard your name.

This is the most diffuse of the three markets, which also makes it the most winnable.

Put those two side by side and you have the whole argument. In home care, national franchises hold the answer. In elder law — same city, same families, same engines — local firms hold all seven top positions. Nothing about being local is the obstacle.

The expensive part

Ninety-seven providers were never named once. Some of them are buying ads.

I checked every provider the engines never mentioned against their own site for advertising and analytics tags. These are organisations already paying to be found, absent from the channel their families now start with.
